California law provides individuals the ability to make their health care wishes known through a form called an Advance Health Care Directive. An Advance Health Care Directive lets your physician, family and friends know your health care preferences, including the types of special treatment you want or don't want at the end of life, your desire for diagnostic testing, surgical procedures, cardiopulmonary resuscitation and organ donation. By considering your options early, you can ensure that your wishes are known and that your family doesn't have to "guess" or make critical medical care decisions for you under stress or while in emotional turmoil. California Advance Health Care Directive combines Power of Attorney for Health Care, a Living Will, and a Directive to Physicians regarding organ donation into one document. However, CA Law does not require you to follow the formula "three documents in one". For example, you may choose to appoint your agent to make important health-related decisions for you in a Power of Attorney for Health Care and not to sign a Living Will. Our website provides you with a choice of purchasing a single document called "California Advance Health Care Directive" that consists of following parts: Part 1 - a Power of Attorney for Health Care; Part 2 – a Living Will (instructions for health care); Part 3 – a Donation of Organs at Death (optional); Part 4 – a Primary Physician Designation (optional); and a Durable Springing Power of Attorney for Property and Finance; The structure of California Advance Health Care Directive is regulated by California Probate Code
