You need to complete the Securities Transfer to Living Trust if you want to transfer ownership of the stocks, bonds, debentures,
treasury bills and notes that you have to the Living Trust. By completing the Securities Transfer to Living Trust you authorize
you agent to change legal ownership name from your personal name to the name of the Living Trust.
The Securities Transfer to Living Trust requires that the signature of Grantor be witnessed by an officer of commercial bank.
Before you transfer the Stocks and Bonds into you Living Trust, contact your broker for additional information and obtain a Stock
Power form from him/her.
Most likely the banks where you have an account, or brokerage firm, have their own similar forms. In this case it is better to
use their form.
