Witnesses and notary. It is generally required to have at least a witness sign your Advance Directives, if not certification by a Notary Public, in order for your documents to be valid. It is easy and should not be expensive to find a notary near you (check your local yellow pages). Many states restrict who may be witnesses on your Advance Directives, and we recommend that you do NOT have a family member, your health care provider or their employees, or anyone who is entitled to any part of your estate. Once completed, signed, and notarized (or witnessed), your Advance Directives will be your Legal document, to be shared with your family and health care provider.
