The structure of the California Advance Health Care Directive is regulated by the California Probate Code.
It defines health care treatment decisions that can be made on an adult person’s behalf.
|
|
|
|
|
The California Advance Health Care Directive combines into one document a Power of Attorney for Health Care, a Living Will,
and a Directive to Physicians regarding organ donation. However, California law does not require you to follow the
"three-documents-in-one" formula. For example, you may choose to appoint your agent to make important health-related decisions
for you in a Power of Attorney for Health Care and not to sign a Living Will.

which consists of the following parts: Part 1 - Power of Attorney for Health Care;
Part 2 - Living Will (instructions for health care);
Part 3 - Donation of Organs at Death (optional);
Part 4 - Primary Physician Designation (optional); and a Durable Springing Power of Attorney for Property and Finance.
