An Advance Health Care Directive means a witnessed written document, voluntarily executed by the declarant in
accordance with the requirements of New Jersey Law. It defines health care treatment decisions that can be made on your behalf.
|
|
|
|
|
According to New Jersey law ("Advance Directives for Health Care Act"), the advance directive for health care may be a "proxy directive"
and/or an "instruction directive." Any competent adult may prepare a written Directive to control the health care treatment decisions
that can be made on that person's behalf.
Our website provides you with a choice of purchasing:
I. a single document called "New Jersey Advance Health Care Directive" that consists of following parts:
(1) an Instruction Directive (known as New Jersey Living Will - instructions for health care);
(2) a Proxy Directive (New Jersey Durable Power of Attorney for Health Care - appointment of agent to make health care decisions);
AND a Durable Springing Power of Attorney for Property and Finance
II. OR, you may purchase some or all of these forms separately.
