According to NY law, an Advance Health Care Directive means a witnessed written document directing that
life-sustaining procedures be withheld or withdrawn. It also defines other health care treatment decisions that can be made on your behalf.

New York Living Will allows you to express your end-of-life health related wishes in advance, while you are in a good state of mind. In a Health Care Proxy you appoint somebody you trust to make end of life decisions for you.
