An Advance Health Care Directive means a witnessed written document, voluntarily executed by the declarant in accordance with the requirements of Virginia Law.
It defines health care treatment decisions that can be made on your behalf.
|
|
|
|
|
According to Virginia law ("Health Care Decisions Act"), any competent adult may prepare a written directive to control the health care treatment decisions that can be made on that person's behalf.
Our website provides you with a choice of purchasing a single document called "Virginia Advance Health Care Directive" that
consists of following parts:
(1) a Living Will (instructions for health care);
(2) a Durable Power of Attorney for Health Care (appointment of agent to make health care decisions);
(3) an Appointment of Agent to Make Anatomical Gift section; and
AND a Durable Springing Power of Attorney for Property and Finance
